Northern Kentucky University provides family and medical leave to eligible employees according to the Family and Medical Leave Act (FMLA). Applicable federal, state and local government regulations may also be applied.
The employee must request a leave from his/her department head and Human Resources 30 days prior to its commencement. If unable to make request in advance due to the seriousness of the health condition, a request must be made as soon as possible after the onset of the condition.
Please turn completed paperwork into Human Resources.
You must provide a return to work certification from your physician to the FML Administrator at least 2 business days before your expected return.
You may not return to work before being released by the FML Administrator.
