Mandatory Psychological Assessment

  1. Counseling Services will accept mandated psychological assessment referrals from NKU Deans (or those administrative personnel with comparable authority). A Mandated (Mandatory) Psychological Assessment Referral Form (doc) must be completed to initiate the referral process and returned to Health, Counseling and Student Wellness.
  2. Counseling Services only accepts mandated psychological assessment referrals that can be completed over the course of one or two sessions. Assessments requiring more than two sessions will be referred to off-campus practitioners.
  3. Counseling Services staff only provides information related to students' mandated psychological assessments to referral sources when students provide written consent. Assessments will not be completed and the student will be referred back to the referral source if the student declines to provide written consent.
  4. A formal letter documenting the assessment will be sent to the referral source and copied to the student.
  5. Referrals will be made to appropriate off-campus practitioners as desired by the student or deemed appropriate by Counseling Services staff.
  6. Counseling Services does not provide services to students who are required to receive mental health treatment or assessment by a court of law. Students seeking such services will be referred to off-campus practitioners.
  7. A copy of this policy is provided by Counseling Services to all students participating in mandatory psychological assessments.

Don't Forget

All Mandated Psychological Assessment Referral Forms (doc) must be must be sent to our office (UC 440) by the Dean or Advisor mandating referral. 

The form cannot be filled out on-line or transmitted electronically because it needs to have an actual signature on it from the person submitting it.
