Thank you for your interest in making a referral to UCAP: University Connect and Persist.
We are encouraging faculty and staff to use the EAB Student Success Collaborative tool for referrals.
If you feel comfortable doing so, please login to nku.campus.eab.com or click on the grey "make a referral' button above to access EAB. Once logged in, search for the student(s) you would like to refer in the top right search bar.
Then on the right-hand side, please click "Issue an Alert" and fill in the information.
Any faculty or staff member may refer a student to UCAP for any reason: academic, personal, financial or other. UCAP staff will reach out to the student. If the student responds, UCAP staff will work with the student to strategize solutions. The student may be referred to other campus resources for additional support.
The chart below is the university's referral guide. It was created by the Health, Counseling, and Student Wellness office to assist faculty and staff members with assesing their student of concerns situation and then referring them to the appropriate place on campus.
If you are ever unsure where to send your student, please feel free to use our referral form (link above) and we will then assess the student's situation and connect them to the appropriate resources.
Our office use Navigate as the system to collect student referrals.
You may submit a referral directly through this link.
This link will take you to Navigate where you can look up the student's profile and submit a referral via the "Issue an Alert" found on the right side of the student's profile page.
*When including comments, please remember that they are part of the student's record and could be viewed by the student and other interested parties.
After you submit your referral, UCAP staff will:
Within 24-48 hours
Within 1 week
Within 2 weeks
*A closed referral means that the UCAP Office will discontinue outreach to that student. As a referrer, you may update UCAP staff about the student's situation and refer a student multiple times if you have additional concerns.
