FAFSA Documents

The FAFSA questions ask for information about you (your name, date of birth, address, etc.) and about your financial situation. Depending on your circumstances (for instance, whether you’re a U.S. citizen or what tax form you used), you might need the following information or documents as you fill out the FAFSA application:

  • Your Social Security number
  • Your parents’ Social Security numbers if you are a dependent student
  • Your driver’s license number if you have one
  • Your Alien Registration number if you are not a U.S. citizen
    • IRS 1040
    • Foreign tax return or IRS 1040NR
    • Tax return for Puerto Rico, Guam, American Samoa, the U.S. Virgin Islands, the Marshall Islands, the Federated States of Micronesia, or Palau
  • Federal tax information, tax documents, or tax returns including IRS W-2 information, for you (and your spouse, if you are married), and for your parents if you are a dependent student:
  • Records of your untaxed income, such as child support received, interest income, and veterans noneducation benefits, for you, and for your parents if you are a dependent student
  • Information on cash; savings and checking account balances; investments, including stocks and bonds and real estate (but not including the home in which you live); and business and farm assets for you, and for your parents if you are a dependent student.

Financial Assistance Forms For The 2022-2023 Academic Year

  • 2022-2023 Additional Request Form
    This form is to be used in the event that you either did not accept all your loan funds and now you wish to take the funds, or you rejected your loan funds and now you wish to take them.
  • 2022-2023 GRAD PLUS Form
    Student MUST be a graduate student or law student to apply for the Graduate PLUS Loan.
  • 2022-2023 Parent PLUS Authorization Form
    Parents of undergraduate students who have been offered the Parent PLUS loan must complete and submit this form in order to apply for this loan.
  • Parent PLUS Loan Eligibility Confirmation Form
    This form and the required documents must be completed and submitted if the parent who is borrowing the PLUS Loan is not listed on the student's FAFSA.

  • Outside Scholarship Declaration Form
    The NCAA has very strict rules about student-athletes receiving outside scholarships. As a student-athlete if you are a recipient of an outside scholarship, it is very important that you contact the Athletics Compliance Office to fill out the Financial Aid Declaration form.
