Quality health care means getting the care you need when you need it.  Getting quality care can help you stay healthy or recover when you become sick. Choosing and establishing a relationship with a Primary Care Provider (PCP) is the key to receiving quality healthcare.  Meeting with them annually to check your health status and discuss your health issues is one of the most important things you can do for your well-being.

NKU cares about your health and well-being and encourages you to take an active role in caring for yourself. By taking the time to meet with your PCP for an annual check-up and biometric screening, all benefit-eligible employees will be rewarded with a $25 All Card gift certificate. Available to all benefit-eligible faculty and staff.


Receiving your reward is simple.

1.     Print your NKU Cares PCP visit form and complete the top portion.
Click HERE to print the form.

2.     Take the form to your annual check-up and have your primary care provider sign and date the bottom of the form and return to you.

3.     Submit your completed PCP form and completed Prize and Award form via email to Kim Baker at bakerk7@nku.edu. Complete all highlighted areas of the prize and award form. Limit one per rolling calendar year, per preventive screening.

Who is a Primary Care Provider?
A PCP is a healthcare provider - a Physician, Nurse Practitioner or Physician's Assistant (PA) - who specializes in internal medicine or family medicine. An internal medicine provider deals with prevention, diagnosis and treatment of adults. A family primary care provider specializes in the care and treatment for children as well as adults. 

Why do I need a PCP, especially if I don’t get sick?
Prevention is the best way to stay healthy. Your PCP can coordinate all of your health care in one place. Individuals with an established relationship with their PCP have better management of chronic conditions, lower healthcare costs and higher satisfaction of care. 

How do I find a PCP?
