The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records. The law applies to all schools that receive funds under an applicable program of the U.S. Department of Education.

FERPA gives parents certain rights with respect to their children's education records. These rights transfer to the student when he or she reaches the age of 18 or attends a school beyond the high school level. Students to whom the rights have transferred are "eligible students" with the following rights:

  1. The right to inspect their education records.
  2. The right to request an amendment to their records if the student believes the records are inaccurate.
  3. The right to control disclosures of their records except to the extent that FERPA authorizes disclosure without consent.

You may, at your discretion, grant the University permission to release information to a third party by submitting a completed FERPA Waiver and Student Information Release Authorization form.  Your records will be made available only if the authorized third party making a request can provide the personal code you have created below.  Anyone who calls or visits the University and provides the code will be given access to the information selected; therefore, it is of the utmost importance that you safeguard this code.  The University does not automatically send/provide information to a third party.  The University will verify your signature and the third party’s identity by the information you release on this form.  The third party representative must present a Picture ID at the time of the request.
