A revision is any planned change to an IBC approved protocol. All revisions must be submitted to and approved by the IBC prior to initiation.
To submit a revision to an approved BAR form, the PI should:
1. Complete the IBC Revision Form.
2. Edit the approved BAR form by:
· Selecting "Revision" at the top of the BAR application.
· If applicable, revising the BAR form to include the relevant edits. All revisions must be highlighted in some way.
· If applicable, complete the Addition of Personnel Form or Addition of Students - Teaching Protocols Form.
3. Submit the IBC Revision Form and the edited BAR to biosafety@nku.edu.
Please note, if the revisions are significant, a new BAR form may be requested.
Though each IBC BAR application is approved for three (3) years, the NKU IBC requires an annual report to be completed per project. Based on the approval date of the BAR, a report will be due by the annual anniversary of approval.

Annual Report, With Revisions
The following steps will be taken if the PI does not submit the annual report:
1. The IBC Administrator will contact the researcher to notify the researcher that the annual report is late (30 days after due date).
2. After another 30 days, if the annual report has not been submitted to the IBC Administrator, the IBC Administrator will contact the researcher and copy the Chair of the IBC and the IBC of the department, or immediate supervisor, on the email (60 days after due date).
3. After another 30 days, if the annual report has not be submitted to the IBC Administrator, the IBC Administrator will contact the researcher and copy the Chair of the IBC, the Chair of the department, or immediate supervisor, and the Dean of the college on the email (90 days after due date).
4. After another 30 days, if the annual report has not be submitted to the IBC Administrator, the IBC Administrator will contact the researcher and copy the Chair of the IBC, the Chair of the department, or immediate supervisor, the Dean of the college, and the Vice Provost for Graduate Education, Research, and Outreach on the email (120 days after due date).
5. After another 30 days, if the annual report has not been submitted to the IBC Administrator, the research study will be placed on an administrative hold until the report has been submitted.
Approved BAR applications are valid for no more than three (3) years. After three (3) years, a new BAR form is required if the researcher wishes to continue the study. Upon re-approval, a new IBC number will be generated and full IBC review completed. Studies cannot be continued without an approved renewal application.
To avoid expiration, the protocol renewals should be submitted thirty (30) days prior to expiration.
